Waiver & Medicaid Support Services

Colorado Medicaid offers several programs—often called waivers—that help individuals with developmental or medical needs receive care and supervision at home. These services are designed to support daily living, safety, and long-term stability while allowing families to remain together at home.

We work with families receiving services through programs such as CES, SLS, DD, CHRP, and related Medicaid supports, including Community First Choice (CFC).

Programs We Support

Families we serve may be enrolled in one or more of the following:

CES (Children’s Extensive Supports)

SLS (Supported Living Services)

DD (Developmental Disabilities)

CHRP (Children’s Habilitation Residential Program)

CFC (Community First Choice) – a Medicaid benefit that provides additional in-home support services

These programs may provide supervision, personal care, daily living assistance, and structured support based on individual needs and authorizations.

How These Services Help

Depending on the program, services may include:

Supervision and safety monitoring

Assistance with personal care and daily routines

Support with transitions and activities

Encouragement of independence and life skills

Consistent, familiar care at home

Services are individualized and guided by each person’s approved care plan.

Parent & Preferred Caregiver Option

When permitted by the program, parents, guardians, or other trusted caregivers may be hired and paid to provide care. This allows support to come from someone who already understands the individual’s routines, communication style, and needs.

All caregivers complete required onboarding and follow approved service guidelines.

Our Role

We help families understand which services they receive, how caregiver options work, and how to maintain structure and consistency so care at home feels manageable and sustainable.

Supporting families through Medicaid waiver and CFC services, including CES, SLS, DD, and CHRP—delivered at home by trusted caregivers when eligible.
